Efavirenz with Tenofovir May Increase Liver Risk

Could the coadministration of efavirenz (Sustiva, Stocrin) and tenofovir (Viread) potentially increase a patient's hepatic risk? Clinicians in northern Italy have reported three cases of HIV-infected patients with otherwise healthy livers who experienced elevations in liver enzyme levels while taking efavirenz with tenofovir. Although the findings are purely anecdotal at this point, efavirenz is known to cause elevated liver enzyme levels in about two percent of patients, and previous case reports suggest that tenofovir may increase efavirenz levels. Efavirenz and tenofovir, of course, make up two of the three drugs in the popular fixed-dose combination pill efavirenz/tenofovir/ emtricitabine (Atripla).
